Tryouts Registration for AC Xtreme Basketball Club 

Tryouts will be held February 23rd, 2025 at Apex Sports Zone, 4565 Elida Rd, Lima.

If you’re unable to make it that day, please select "need schedule for makeup date below" and you will receive emails with a make up date. Per OHSAA rules, players in grades 7th-12th must be completed with their school sports season before trying out for club.

February 23rd tryout schedule:

5-6PM: 3rd-4th Grade, boys and girls

6-7PM: 5th-6th Grade, boys and girls

7-8PM: 7th-8th Grade, boys and girls

8-9PM: 9th-12th Grade, boys and girls

Waiver/Consent for Medical Treatment / Agreements
By registering, you are agreeing to the following terms and conditions, I release my child to participate with AC Xtreme Basketball Club. I recognize that my registering on this website, this release is a condition of your permitting my child to participate. I certify that my child is in excellent physical health, and may participate in physical activities. I certify that there are no physical limits to my child's participation in the camp. Permission is granted for my child to receive emergency medical treatment if needed. I hereby release and discharge AC Xtreme Basketball Club and Apex Sports Zone, and all their affiliated entities from any and all liability, claims, demands, and causes of action for personal injury, property damage, and/or other loss suffered by my child in connection with his/her participation in the camp. I represent that I am a parent/guardian of the minor named below and I agree that the grant and release contained therein binds me and the minor to all its terms. I do hereby authorize AC Xtreme Basketball Club and Apex Sports Zone and its assigns to utilize any and all photographs, pictures or other likeness of my child or any participant assigned guardianship to me, as they deem appropriate in its promotional materials.. I understand that after I register, there are no refunds given.   There will be no refunds given unless we do not have enough players for a team. I understand that the coaches of the team will determine which events to attend.
